You can email the Youth Office using the link in the Activities menu top left.
Use this link to download a copy of the parental consent form. If you wish, you can save a copy to your computer and enter the main details. When asked for a consent form, add the specific activity and date, print out a copy, sign it and send it in. Please note a separate consent form MUST be given for each activity. |
