Medical information 3A- This is to be completed for long term medication such as epi-pens, asthma pumps or medication to be retained by the office for emergencies or occaasional use such as paracetamol, ibuprofen, piriton etc for headaches, period pains or allergies.

Medical information 3B - This is to be completed for antibitotics and any short term medication.

If students have a change of medical circumstances or the school needsto be informed if they need to carry medication on them long term (ie epi-pens and those with diabeties), then they need to come to the school office to complete a health care plan.
